Request for an Appointment / Site Visit

  1. Accomplish the SMMCI Visitors Form below
  2. You can only book an appointment one week before your intended visit.
  3. Wait for confirmation from the SMMCI Management for the final schedule of appointment/site visit, and approval of your request.
  4. Upon approval you will be issued a digital approved pass – please print or save a digital copy on your phone, and have it with you at all times. This will be presented to the security at the main gate, and to office/person you are visiting.
  5. Make sure to read and understand SMMCI Site Guidelines below to ensure smooth entry at the site premises.

Site Guidelines

  1. Bring one (1) Valid ID. NO ID, NO ENTRY. All visitors of SMMCI are required to present a valid ID before entering the terminal.
    • If from a company, please bring a Company ID
    • If not employed to a company, please bring any Government ID
  2. All visitors should observe proper dress code, the following attire will NOT be allowed to enter the premises of SMMCI:
    • Sando, strapless blouses (unless worn as an undershirt), walking shorts, sandals, slippers or any open toe shoes.
  3. Only those listed on the visitor’s form will be allowed to join the site visit/tour unless a new form is filled out and submitted prior to the scheduled tour.
  4. For arrivals/drop-offs: For all our guests, please proceed to the Main Gate for security validation and inspection. They will advice you where to go, and where to park your vehicles.
  5. For late arrivals: Thirty (30) minute grace period can be extended for late arrivals. For arrivals beyond the grace period, tour will be cancelled unless allowed to push through upon the discretion of the tour facilitator
  6. For tour cancellation:Cancellation of tour must be made at least two (2) days prior to scheduled tour.

Security and Safety Regulations

1 Security verifications will be conducted at the main gate.
2 Security reserves the right to determine the parking area, search the vehicle and adapt all other security responses as deemed necessary.
3 Nobody under the influence of liquor and/or drug will be permitted to join the visit/tour.
4 Carrying of deadly weapons is not allowed.
5 Visitors are not allowed to enter areas that are not part of the approved itinerary.
6 Guests and visitors are not allowed to go alone without a guide nor linger in any part of the worksite during and after the end of visit.
7 Do not engage in any behavior, including horseplay that may endanger yourself or others.
8 Visitors are not allowed to enter areas that are not part of the approved itinerary.
9 Smoking is only allowed on designated smoking areas.
10 Visitors are strictly prohibited from taking photographs and video footages at any part of the mine site – unless permitted by the host department/person. You will be held liable for any unauthorized posting or publication of photographs/vide footages.
11 PERSONAL PROTECTIVE EQUIPMENT shall be issued by the Safety Custodian and shall be cleaned and returned after use.

Legal Form Consent

To Processing of Personal Information

Being a/an applicant, customer or supplier of, or any other third-party transacting or dealing with, SILANGAN MINDANAO MINING CORP. INC. (“SMMCI”), affirm that:  

  • SMMCI may process information about me in relation to my position. Information processed may include: name, marital status, Tax Identification Number, age, address, education, profession, business experience, business affiliation, family affiliation, number of shareholdings, directorship in other companies, government service, any information from which identity of an individual is apparent or can be reasonably and directly ascertained, whether recorded in material form or not, such as CCTV footages and recordings within SMMCI premises and other information which may be considered personal and sensitive personal information (collectively, “Personal Information”) under the Data Privacy Act of 2012. Processing is any operation performed on the Personal Information, such as, but not limited to, collection, retention, disclosure, publication and destruction (“Processing”). 

  • The Processing of my Personal Information is for purposes of: 
    • Identification in company records and corporate housekeeping;
    • Corporate transactions and business operations;
    • Compliance with the requirements, including reportorial obligations, by the Security and Exchange Commission (SEC), the Philippine Stock Exchange (PSE), Bureau of Internal Revenue (BIR), relevant local government units, and other appropriate government agencies or offices; 
    • Maintenance of security within and around the premises
    • Compliance with legal processes or lawful orders of judicial and quasi-judicial bodies, or any other government agencies and instrumentalities; or
    • Any other legitimate business activities of SMMCI. 
  • SMMCI may share or disclose my Personal Information to:
    • The SEC, PSE, BIR, relevant local government units, and other appropriate government agencies or offices;
    • Any judicial and quasi-judicial bodies or government agencies pursuant to a lawful order; c) Any customer or port user; 
    • Any agent, contractor, consultant, adviser, auditor, underwriters or any service provider SMMCI engages to carry out legitimate business activities or services;
    • Any lenders, creditors, or other counterparties in legitimate business transactions; or
    • Any other persons as may otherwise be considered necessary and appropriate by SMMCI for its legitimate business activities. 
  • Those enumerated in Clause 3 above may also need to continue the Processing of my Personal Information for the same purposes enumerated in Clause 2 above and/or other lawful purposes.

  • The Data Privacy Act of 2012 prohibits the Processing of Personal Information without the consent of the data subject, unless required by law.

I hereby freely give this specific and informed consent for the Processing of my Personal Information in accordance with the above. This consent is valid while I am a/an applicant, customer or supplier of, or any other third-party transacting or dealing with SMMCI. However, I understand that I can withdraw this consent anytime by informing SMMCI in writing.
